a Wellthwise Program can be modified to suit your needs. One-of appointments
are also welcomed.
New Wellthwise
Program customers are requested to complete the in-depth
online Health Assessment Form
at least 2 days prior to the first appointment time.
Allow 1 (one) hour to complete
the on-line form. This form can be accessed at www.metagenics.com
and the graph displayed at the end of the questions will give BOTH you
and Vivien a clear indication of your immediate health priorities and
where your health dollars will be most effective. Directions for giving
you access to the Health Assessment Form will be emailed to you after

Additional Tests
In some cases, additional tests may be required and these are charged
at the recommended retail prices – examples of external tests
can be viewed at www.arlaus.com.au.
They may be saliva, urine or stool testing, and prices include all courier
charges. External tests are pre-purchased separately through Mosman
Naturopathic Practice, and sent to your home in convenient kit form.
